Job Description

Job Summary:

The Case Manager RN coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.



  • Creates plan for care across the continuum, integrating patient/family preferences and values.

  • Monitors patient care through assessments, evaluations and/or patient records.

  • Advocates for resources and removal of barriers.

  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

  • Hospital - Coordinates and integrates effective utilization management through chart review and interdisciplinary team collaboration to ensure appropriate resource utilization. Ensures regulatory compliance with CMS guidelines.SCLM - Applies advanced critical thinking and conflict resolution skills using creative approaches.

  • Hospital - Demonstrates professional communication skills to foster an interdisciplinary team approach to patient care, ensure adequate documentation, and effectively respond to diverse cultural needs of patients/families.SCLM - Demonstrates positive and professional written, verbal, and non-verbal communication skills.

  • Hospital - Utilizes advanced critical thinking skills to assess patient/family needs, problem solve, and promote cost effective management of resources.SCLM - Enhances professional organizations, department activities, and community groups.

  • Provides information and education to promote access to available resources and to ensure patient/family have necessary tools to make informed decisions.



  • Required Credential(s):

    • One or more of the following:

      • Licensed?Advanced Practice?Nurse Practitioner?credentialed from the?Florida Department of Health?obtained prior to hire date or job transfer date.?

      • Licensed?Registered Nurse?credentialed from the?Florida Department of Health?obtained prior to hire date or job transfer date.?


  • Associate of Science?

    • Associate degree in Nursing required; Bachelor's degree in Nursing (BSN) preferred

Work Experience:

  • 3+ years nursing experience required; acute care or case management experience preferred.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
